

As therapists, we know pain rarely lives where it appears.
What matters most is understanding the patterns underneath the tension.
Here are the five patterns I look for in every client and the ones I see most often in clinic.
1. Under-active Deep Core → Overactive Lower Back
When the deep core isn’t supporting, the lumbar spine does the stabilising.
What you’ll see:
tight lower back
hypersensitive QL
rib flare
clients “hanging” into extension
Why it matters:
Without core support, no amount of back release will hold.
2. Locked Hips → Overloaded Glutes + Hip Flexors
When hip mobility is restricted, surrounding muscles grip for stability.
What you’ll see:
tight hip flexors
“dead” glutes
clients who stretch but stay tight
tension dumping into the SIJ
Why it matters:
Restricted hips change everything above and below.
3. Shallow Chest Breathing → Neck + Shoulder Tension
If the breath can’t drop into the ribs, upper-body muscles take over.
What you’ll see:
tight scalenes and pecs
shrugging shoulders
forward head posture
difficulty relaxing during treatment
Why it matters:
Correcting breathing often reduces neck tension instantly.
4. Stiff Thoracic Spine → Compensations Through Hips & Back
A locked mid-back forces the lower back and hips to move excessively.
What you’ll see:
limited rotation
flat thoracic curve
“stuck” ribcage
clients complaining their hips won’t loosen
Why it matters:
You can’t change pelvic mechanics without thoracic mobility.
5. Stress-Driven Guarding → Full-Body Tension
The nervous system holds more tension than any muscle.
What you’ll see:
bracing
shallow breath
difficulty letting go
persistent tightness despite good mobility
Why it matters:
If the system doesn’t feel safe, the tissue won’t release.
A Therapist Note:
Patterns tell us more than symptoms ever will.
When you work with the body’s natural responses — not against them — your treatments hold longer, clients understand their bodies better, and results improve dramatically.
